This notice explains how we handle information you send through public website forms (contact and newsletter). It does not replace our full clinic Notice of Privacy Practices (NPP) for protected health information (PHI) under HIPAA. Request the full NPP at the front desk or by phone.
Please do not submit through website forms: detailed medical history, Social Security numbers, credit/debit card numbers, insurance member IDs with sensitive claims data, or other PHI. Clinical and payment information is collected securely at the clinic.
We do not sell your contact information.
Website forms are processed through an email delivery service to [email protected]. This public web channel is not described as a BAA-covered PHI transmission portal. That is why we ask you not to include PHI in website messages.
As a dental care provider, LeesPike Dental follows applicable U.S. health information privacy laws for patient care records. Website form data is treated carefully for clinic operations, but this page is only a website notice. For rights and practices related to your dental chart and PHI, see our clinic NPP.
The site may use technical cookies or local storage required for basic operation (for example, navigation and form helpers). If analytics or marketing tools are added later, this notice will be updated.

Contact and newsletter messages are kept only as long as needed to respond, schedule care, or send requested updates, unless a longer period is required by law or legitimate clinic records needs.
